Provides person-centered individualized services and supports to people with I/DD to promote independence, productivity, integration and inclusion in their communities. Creates connections to natural supports in their community for activities and resources that ensure a person’s overall well- being. Assist each individual in meeting their personal life goals. Identifies and resolves problems and barriers; documents progress and observations; provides feedback to the circle of support.

Requires maturity, dependability, excellent communication skills, and must be proficient in the use of computers and other electronic devices such as smart phones, and tablets. Required to use personal vehicle for company business and to transport participants. (Must be at least 25 years old.)
